Salary Range: $58,000 – $80,000 annually (commensurate with experience)
Position Summary:
We are seeking a compassionate, dedicated, and skilled Social Worker to join our team in Massachusetts. The Social Worker will provide comprehensive psychosocial support services to individuals and families, ensuring the highest quality of care and compliance with state and federal regulations. This position plays a critical role in supporting residents’ emotional well-being, care planning, and access to community resources.
Key Responsibilities:
- Conduct psychosocial assessments and develop individualized service plans
- Provide counseling and emotional support to residents and families
- Assist residents with care coordination and discharge planning
- Connect individuals and families with appropriate community resources and services
- Maintain accurate and timely documentation in accordance with regulatory standards
- Participating in interdisciplinary team meetings
- Advocate for residents’ needs and rights
- Ensure compliance with Massachusetts state regulations and professional standards
Qualifications:
- Bachelor’s degree in social work (BSW) required; master’s degree in social work (MSW) preferred
- Current Massachusetts social work licensure (LSW or LICSW preferred) or license eligibility
- Bilingual in English and Chinese preferred
- Minimum 1 year of experience in long-term care environment
- Strong assessment, communication, and organizational skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of Massachusetts social service systems and community resources

Positions: Part-Time
Job Description: Provide primary nursing care to patients, by administering medications, recording and evaluating patients conditions, and reporting symptoms or changes; generate care plans to improve or maintain patients physical functioning; revise care plans based on changes in patients responses and conditions; monitor flow sheet charting and progress notes; conduct nursing diagnoses based on signs and symptoms recognized; communicate information about patients to physicians; document and update patient medical records in accordance with specific policies and procedures; explain instructions and care plans to patients; assign tasks to nursing assistants based on the needs of patients; instruct nursing assistants in proper procedures and complete performance evaluations.
Minimum Requirements: Graduate of accredited RN school of nursing; valid RN license in the Commonwealth of Massachusetts.
Positions: Full-time, 40 hours per week, 3pp-11pm Shift
Job Description: Plans, coordinates and provides total nursing care for residents and provides supervision and guidance to clinical staff members.
Minimum Requirements: Graduate of accredited LPN School of Nursing; valid LPN license in the Commonwealth of Massachusetts
Job Description: Provides basic nursing assistance and assists residents with daily living activities.
Requirements: must have 6 months long term care experience.
Qualifications: High school diploma or equivalent; Current certification as a CNA in the Commonwealth of Massachusetts.
